1) PERSONAL METHOD
Be a qualified SEMD by June 30, 2013* AND personally enroll 12 people with CEO MEGA PAKS during the contest period.

2) GROUP METHOD
Be a qualified SEMD by June 30, 2013* AND have 39 new enrollments with the CEO Mega Paks on levels 1 thru 3 during the contest period (3 of which must be YOUR personal enrollees).
